This questionnaire does not ask for any private information except in the broadest terms. Only when professional instructions are in place, will we ask for confidential information. It should only take you a few minutes to complete.

Filling in this questionnaire helps us understand what assistance you require, and we will tailor our reply to what you tell us. You should answer every question 'Yes' or 'No' by filling in the buttons. All the boxes are already filled in with default answers. Please double check that every answer is correct before submitting the form.

UTR – Unique Tax Reference
